Worker’s Compensation

The local benefit coordinator is the first point of contact in the event an employee has an on-the job injury.  The Work Comp Grab and Go Kits below contain a checklist and the necessary forms to report the injury.  Once completed, all forms must be submitted to the Diocesan Human Resources Office for processing.

Grab n Go: First Report of Injury (FROI) (use for injuries requiring medical treatment with a physician or medical provider)

Grab n Go: Incident Report (us for incident only reporting, no treatment sought)

Effective July 1, 2016, Worker’s Compensation Claims for the Catholic Diocese of Sioux Falls are processed through Accident Funds and administered by Howalt McDowell Insurance, a Marsh & McLennan Agency, LLC.
